Halloween's coming up Friday, and what better way to bring up an unintentionally scary game?
Recently, I've been playing and listening to a lot of Super Mario 64, and it occurred to me I was very scared about playing this game as a kid, and quite frankly still am.
Several reasons for that:
1. Falling: Falling into bottomless pits and the sky scared me as a kid because there wasn't any fanfare "death" music in SM64. Here, the game continues playing the same music, as if you're not good enough to be treated to special music. Especially creeps me out in the Bowser levels
2. The Eel: I rarely played in Jolly Roger Bay, because the eel that hides in the ship made me crap my pants when I was young. I often begged my brothers to play the levels for me because that Eel looked scary as hell. (Obviously now I can play it with little problems, but back then, I was traumatized)
3. Piano: Boo's Mansion has this monstrosity, and yeah, you know the rest. Try to get a red coin, the piano comes alive and vicious teeth.
4. The Giant Fish From Little Big-Island: You'll notice a giant fish in the water in this world. Go next to it, and it eats you alive in one bite and you die. The way it happens seems silly, but back then I turned the game off. Never been so scared in my life.
5. Whirlpool: Played the mission in Dire Dire Docks where you have to avoid a whirlpool. Got sucked in it, was the last time I ever played that level.
6. Endless Staircase: No comment.
I sound like a child, but those things truly scared me when I first played SM64 back in the N64 days. So yeah Happy Halloween.

This has more to do you with how young you were and your own imagination
than with the game itself, even though I seem to remember a similar Mario 64
thread a few years back.

The piano in Boo's Mansion is a little creepy and can kick your ass, and I can understand how that and the whole level in general could be scary to a little kid. But I don't find any of the other examples scary.

I never really caught any of that, but then again, I played this during the height of my stoner days, so what do I know?
