Minigolf Popup book?

Trying to remember the title to a book I had as a kid. Each two-part spread was a mini-golf hole, with pop-up bridges & obstacles. It came with cardstock clubs & foam balls; over time, the balls got smashed by the book.

Last hole was a desert with an abandoned building & snakes. There was also a castle wall with a bridge over crocodiles somewhere in the book.

Anyone remember anything like this?
