Terrifying in terms of jump-scare factor, terrifying due to how bad it is, or terrifying because of reality/implications?

Forgot to mention one other frightening foe from the first Splatterhouse: The Biggy Man! I mean come on, giant with chainsaws for hands and a bag over his head, eat your hearts out Leatherface and Jason!

Amnesia: The Dark Descent was pretty scary,and in fact so scary for my wife that she had to quit playing it after about 30 minutes. She couldn't handle the tension!

My vote is for Dungeons of Daggorath on the Tandy Color Computer (CoCo), though the site doesn't cover CoCo games.
I've seldom if ever played a game so nerve-wracking and so stressful. Your health icon is literally a heartbeat, and goes up as you take damage or exert yourself! You even go into fibrillation when your character is knocked unconscious or dies! It gave me nightmares when I was a kid...
Outside of that, I think the homebrew Skeleton+ on the Atari 2600 made me jump a couple of times when I played it years ago.

I also forgot to mention the original Manhunt on PS2. The sequel sucked. But the original what is a heart racing thriller with a lot of brutality, in my opinion somewhat too much, but the original was a lot of fun an extremely tense.

If you think that jump scares are what makes games scary, you should play some scarier games.
