Easily the freakiest boss fight in gaming. Even Majora's Wrath wasn't this scary, and that was the final form!

While not as scary as the Resident Evil series, I could not forget about the House of the Dead series:

HOTD 1: "Revenge" and "Truth" - despite the funky beat underneath both of these is a creepy vibe.

HOTD 3: "Theme of Sun and Fool" - How appropriate for those two bosses, considering the slithering creepiness of the first foe, and the major intensity of the latter.

Oh yes, on the Gameboy, that would probably be the scariest music, so simple yet so effective in bringing the chills! The echo of Tower amplifies the fright!

I know PC games are off topic but for me, Phantasmagoria has a strikingly haunting soundtrack. What starts out playful and melodic gets more off-key and foreboding as the game progresses.

On the console side, I'd go with Mansion of the Hidden Souls (Sega CD). I always found this soundtrack ominous,

I can never remember the actual name for it but the "music" that plays when Nemesis is nearby is the scariest **** ever.
